Rates:

I offer sliding scale opportunities for those individuals that do not have insurance or who are struggling with high deductible insurance plans or who simply choose to not utilize their insurance coverage at this time. Please inquire specifically with me to discuss sliding scale rates.

Insurance:

Services may be covered in part or in full by your insurance carrier. I am currently contracted with Anthem, Husky, Cigna, United Behavioral Health, Optum, Oxford, Connecticare and Healthy CT.  I can work with other insurance companies as an out of network provider, but it is recommended that you contact your insurance company and find out about their out of network provider options before scheduling your first session. I am happy to help with this process.

You may need to inquire through your insurance provider about if you have mental health coverage, deductibles, or if you need a referral from primary care physician. Please contact your insurance company and have an understanding of your benefits prior to our first session but do know that I can help with this process.

Payment:

I accept cash, check, and major credit cards. You are responsible for any returned check fee’s. Payment is due at time of session.

Cancellation Policy:

I understand that sometimes unavoidable circumstances can arise however I ask that you give me as much possible notice so that I can re-arrange my work schedule. It may not be possible to re-schedule an appointment within a quick time frame. Missed appointments or cancellations without 24 hours may be charged a cancellation fee which is not covered by insurance. Medical emergencies or dangerous weather are exceptions to this.
